Instructions for completing the PAIS Employment Application

• Before you begin, please note that the PAIS Employment Application is a .PDF file.  Adobe Acrobat is required to open and complete the application.  Click here to download and install Adobe Acrobat Reader.
  
  
• Please complete all of the fields highlighted in blue on both pages of the Employment Application.  Incomplete applications will not be processed.
  
  
• Once you have completely filled out the application, please click the green "Submit Form" button located in the bottom right hand corner of the Employment Applicaton.
  
• A Security Warning box will appear with a message informing you that the document is attempting to contact http://pais.paiswv.com/PAISReceivePDF.ReceivePDF.aspx.  Please click "Allow".
  
  
• Your will receive a message confirming submission of your Employment Application.
