Position: Licensed Practical Nurse

Type:  Part time (10-20 hours per week)

Shift:  Between 7AM - 9PM (flexible)

Days:  Monday through Sunday

Area Served: Beckley & Oak Hill

Pay Range: $13.00-$15.00 per hour

Special Requirements:  LPN must be able to travel to both Beckley & Oak Hill.

Education Requirements:

Posses a current, active West Virginia license in good standing to practice as a Licensed Practical Nurse (LPN).

Responsibilities include:

1. Face-to-face assessments or evaluations of a client. For example, rash, recent injury, cold/flu symptoms, complaints of pain, possible side effects or adverse reactions to medications.

2. Addressing a participants’s medical needs over the phone. For example, support staff may call the nurse to report a client fell, vomited, or had a seizure. The nurse over the phone, assesses the situation and gives directions to the support staff.

3. Implement and supervise all nursing related habilitation program including self medication and related programs.

4. Administer medications and document administration as required.

5. Monitoring of MARs and medication storage.

6. Document all services provided through a progress note.

7 Ensure physician orders are current, properly documented and communicated to direct care staff and others.

8 Monitoring of client medical equipment for safety and proper functioning.

9. Maintain current emergency contact sheets.

10. Other duties and responsibilities as assigned by supervisor.
